Digital Privacy Authorization Release Form

The Digital Privacy Authorization Release Form is a House hosted form that allows constituents to digitally sign a privacy authorization release. Submitted forms will automatically be added to your Activity Inbox in Fireside on the My Cases page, where you can then start a new case.

What Will Constituents See?

After constituents enter a valid in-district address, they will be brought to a standard web form that includes the information you need to start and work on a case. If the constituent's address is in another district, the form will direct them to the appropriate office.

Customized fields for each Agency

When the constituent chooses an agency from the drop-down box on the form, depending on which agency they select, there may be additional fields added to the form. Some agencies cannot work on a case unless they have specific information that other agencies don’t require. By having these customized fields, caseworkers do not have to get that additional information later and they are able to start working on the case right away.

What Will My Experience Be?

Notifications

When the form is configured, your office can choose which email address will be sent notifications. This email will receive updates when the form has been submitted.

Activity Inbox

Once the form has been submitted, the new requests will be forwarded into the Fireside Casework Activity Inbox (Casework > My Cases > Activity Inbox) with the subject “Digital Privacy Release Form”. To see a preview of the submission, click on the arrow to the left of the subject line. From here you can download the file directly by clicking on the file name in blue (Privacy Release Form.pdf).

Add to case

To create a case from the form, click anywhere in the blank space of the line > snowman button on the right side > Create a Case.

From here you will create the case as you normally would. The digital form will added under “Files” in said case and a note will be added at the start of case history.

Delete from case

If you would like to delete the submission instead, you will go to the snowman button on the right side of the item > Delete this item.

What Will Agencies See?

Customized PDF

When a form is submitted you will receive a PDF that you can then forward to agencies. This PDF has been customized to contain the specific information that each agency needs.
